Rhinoplasty-Nose Surgery

This word is derived from the language Greek. Greek: rhinos,
(“nose” + plastikos, “to shape”) is a type of plastic surgery that
is used to improve the function or appearance of a person’s nose.
Rhinoplasty is also commonly called a nose job.
This surgery is performed to meet aesthetic goals or for reconstructive
purposes to correct birth defects or breathing problems. It can be
combined with other surgical procedures such as chin augmentation to
enhance the aesthetic results.

History:
The Rhinoplasty was first developed by Sushruta. He was a very
important physician who lived in ancient India circa 500 BC, which he
first described in his book Susrutha Samhita. He had many students and
disciples who later used Rhinoplasty to reconstruct noses that were
amputated as a punishment for crimes. He developed techniques of
forehead Flap Rhinoplasty which
is practiced almost uncha nged to this day. This knowledge of plastic
surgery existed in India up to the late 18th century.
The first intranasal Rhinoplasty in the west was performed by John
Orlando Roe in 1887. In 1898 Jacques joshep used it for cosmetic
purposes to help those who felt that the shape or size of their nose
caused them embarrassment and social discomfort.

Associated health risks:
Because of the special nature of the blood supply to the human nose and
surrounding area it is possible for retrograde infections from the
nasal area to spread to the brain. For this reason, the area from the
corners of the mouth to the bridge of the nose, including the nose and
axilla, is known to doctors as the danger triangle of the face.

How Rhinoplasty is performed:

1. It can be performed under a general anesthetic or
with local anesthetic, depending on patient or doctor preference.
2. Incisions are made inside the nostrils. Sometimes
tiny inconspicuous incisions are also made on the columella, the bit of
skin that separates the nostrils.
3. The surgeon first separates soft tissues of the
nose from the underlying structures, then reshapes the cartilage and
bone causing the deformity.
4. In some cases the surgeon may shape a small piece
of the patients own cartilage or bone to strengthen or increase
the structure of the nose
5. Sometimes this is done for cosmetic reasons
6. It can also be done to improve breathing and
function of the nose.
7. In rarer cases, a synthetic implant may be used to
reconstruct the nose if the normal structure of bone and cartilage is
badly damaged or weakened.
8. Alloplastic synthetic materials are often
associated with long-term complications.
9. Cartilage from the septum, ear oro rib may be used
alternatively.
10. To improve nasal breathing function, septoplasty
may be performed, with or without cosmetic changes. The cartilage
removed may be used as a graft to improve the appearance and structure
of the nose.

Entertainment industry:
We all know that the cosmetic surgery is very common is the
entertainment industry. Rhinoplasty is mostly performed on women.

The Los Angeles examiner of May 5, 1930, stated that:
“Having one’s nose shaped to fit the
talkies is the most popular thing in Hollywood now. Hollywood plastic
surgeons agree that more than 2000 facial beauticians operations have
been performed among film players within the past few years.

Introduction:
People young and old undergo this neck lift surgery all the time.
A neck lift will make a person look like he or she has lost a lot of
weight. This procedure corrects the loose neck muscles. Removes the
excess fat and also removes excess skin from the neck.
A neck lift has two kinds of procedures to improve the appearance of your neck.
• Cervicoplasty: this procedure is used to remove excess skin.
• Platysmaplasty: this procedure removes or alters neck muscles.

Pre-surgery planning:
• The neck lift surgery takes two or three hours. The
time will vary according to what kind of surgery you will be going
through.
• In order to undergo a neck lift, you must be in a
good general health, both physically and mentally.
• Be frank with your neck lift surgeon about what you
want to be altered in your neck. Make sure you get the results you want.
• During your consultation, your surgeon will
instruct you on what medications to avoid prior to your surgery.
• If you are a smoker, you must quit at least two
weeks before the surgery and two weeks after, to ensure proper healing.
• Make sure you wear loose, comfortable clothing on
the day of the surgery.

Neck Lift Surgery Procedure and
Problems:
The most common problems are
• Turkey wattle neck weakened or loose neck muscles.
• Excess fat
• Too much skin

muscles
If you have weakened or loose muscle, this may cause the turkey wattle
or appearance of neck “bands”.
Your surgeon may recommend Platysmaplasty surgery to
correct this. The surgeon makes incisions under your chin and/or before
your ears to access the platysma and manipulate it accordingly. The
surgeon may also use permanent sutures to hold the tissue in place.
The surgeon may also decide to use a less invasive procedure by making
a smaller incision and use an endoscope (small camera attached to a
think tube) to do the surgery. You can ask your surgeon to use either
local or general anesthesia depending how you feel.
If necessary botox injections are used to make you feel relaxed parts
of neck muscles which give “band” appearances to the neck. This can be
done on an outpatient basis and completed within 15 minutes.

Excess fat:
During liposuction the surgeon will make a small incision below the
chin and remove the excess fat. This procedure will take an hour. Your
incisions will be stitched and bandaged.

Too much skin:
Surgeon will make sure similar incisions for a Cervicoplasty, trimming parts of
the skin and lifting it into place.
It will be secured with tissue glue or stitches. The Cervicoplasty can
be completed in about two or four hours, depending on the complexity of
your procedure. Again your anesthesia will depend on your own comfort level.
The bandage will have to be worn for a week. Unless instructed
otherwise.

Recovery:
Recovery takes time. Most people return to their normal routine after
10-14 days. You need to wait for at least three weeks before you become
physically active.

Complications:
• In the first few weeks after the surgery you will
have swelling and bruising that can last for several days.
• You may feel tightness or tingling and different
sensations including burning or pulling.
• You may also experience numbness
• There is a risk of infection
• If you are running temperature then contact your doctor.

Face lift is actually known as rhytidectomy. As people age, the effect

Risk of getting a face lift done:
As we all know every surgery has risks involved. It can best know when
discussed with your surgeon. It depends mostly on the person’s anatomy
and their physical reactions, and their healing abilities.
1. Blood collects under the skin must be removed by
the surgeon.
2. Injury to the nerves that control facial muscles.
3. Infection and reactions to the anesthesia.

Planning for your face lift surgery:
You should always discuss your goals with your surgeons. They will

1. Your surgeon will check for medical conditions

tendency to form excessive scars.
2. Your surgeon will instruct you on taking or

weeks before and after surgery.

Types of anesthesia done during Face
Lift :
1. Local anesthesia: it may make you drowsy. You’ll
be awake but your face will not be in pain.
2. General anesthesia: if this anesthesia is given
then you will sleep through out the day.

About the face lift surgery:
1. The face lift surgery takes several hours to be
done with.
2. Incisions usually begin above the hair line at the
temples, extend in a natural line in front of the ear, then it
continues behind the earlobe to lower scalp.
3. The surgeon separates the skin from the fat and
muscle below.
4. Fat may be trimmed or suctioned from around the
neck and chin to improve the contour.
5. The surgeon then tightens the underlying muscle
and the membrane, pulls the skin back, and removes the excess.
6. Stitches secure the layers of tissue and close the
incisions.
7. Metal clips may be used to the scalp.
8. After surgery a thin drain tube is placed under
the skin behind your ear to drain any blood that might collect there.

After your surgery:

1. If there is discomfort after the surgery, pain
medication is given.
2. If there is severe chronic pain or sudden swelling
of your face, its best to alert the doctor.
3. Their maybe drainable tubes and they will be
removed one or two days after surgery.
4. Bandages are usually removed after five days.
5. Your face may look pale bruised and puffy, but in
a few weeks you will be looking normal.
6. Most of your stitches will be removed after about
ten days. Your scalp may take longer to heal, and stitches or metal
clips in your hairline could be left in a few days longer.

Care after the surgery:
1. You can move about in two days but take it easy
for the first week after the face lift surgery.
2. Be very gentle with your face and hair, sine your
skin will be both tender and numb.
3. It may be very numb at first.
4. Avoid strenuous activity, including sex and heavy
housework, for at least two weeks, stretching ise fine.
5. Get plenty of rest and allow your body to spend
its energy on healing.

Your new look:
It’s very common to still have swelling for weeks after surgery but it
will subside with time. You will be happy with your new and improved
look at about three to six months once the swelling is gone.
